To participate in camp, it is required that each camper provide a Waiver and Release of Liability form.  Forms maybe submitted at check in, or faxed to 612-330-1372!

The waiver and release of liability form must be signed by a parent or guardian if camper is under 18 years old.  Campers who are 18 years old or will become 18 years old before the end of the camp/clinic must also sign. In order for campers to participate in camp activities, we must have this form prior to the camp’s start date.
